  10. Your qualifications for the new job position, including degrees and licenses
  11. Are you related to—or would be considered a personal associate of—anyone in the hiring department at stake?
    • Yes
    • No
  12. If yes, inform that person’s name.

Supervisor approval

  1. Do you approve this job application?
    • Yes (move card to ‘HR information’)
    • No (move card to ‘Unapproved applicants’)
  2. If not, specify the reason.
  3. If yes, write your recommendation about the employee

HR information

  1. Date when the application was received by HR
  2. Was the employee interviewed?
    • Yes
    • No
  3. Interview date
  4. Interview status/remarks

HR approval

  1. Has the applicant been selected for the job?
    • Yes (move card to ‘Approved applicants’)
    • No (move card to ‘Unapproved applicants’)
